Below are some our hospital forms. Please familiarize yourself with them ahead of your appointment. Filling out the appropriate forms ahead of time may help facilitate a quicker check in and/or appointment. Boarding Authorization Form	You can fill out this form ahead of dropping your pet off for his or her boarding appointment for a faster check in.
Drop Off Questionnaire	Any time a pet is dropped off, we ask that owners fill out this form. Please fill it out and bring it with you to facilitate a quicker check in.
Medical Authorization Form	We ask owners to fill out this form any time a pet is admitted to our facility for treatment.
Medical Authorization while Owner is Away	If you pets are going to be in someone else's care while you're away, and you'd like to authorize Lyndon Veterinary Clinic to provide treatment as necessary, please fill out this form and return it to our office.
Medical Record Release Form	Please fill this out and return it to our office to authorize the release of your pet's medical records.
New Client Form	You can fill this form out and bring it with you to your first appointment with us for a quicker check in.
